What operations are performed for ovarian cancer?

Surgery is a key way to fight ovarian cancer. It is used for women with early and advanced stages of the disease and for patients whose cancer has returned after previous treatment. 

 

During surgery, general anaesthesia is administered. The tumour is then removed laparoscopically (through 3-4 incisions on the abdomen up to 1 cm in length) or laparotomically (with a vertical incision from the navel to the bikini line). Minimally invasive surgery is about 20-30% more expensive than open surgery.

 

Resection of the tumour using the da Vinci robotic system is also possible. This intervention includes less blood loss and stress for the body but costs more than conventional laparoscopy. The risks of complications after robotic tumour removal are minimal.

 

There are several types of surgical interventions for this disease. In most cases, the surgery involves the removal of the body and cervix, as well as the fallopian tubes and ovaries. These are total hysterectomy and bilateral salpingo-oophorectomy. If the cancer is found early and in only one ovary, some young women who want to have children later in life may have only one ovary and fallopian tube removed. 

 

The choice of a particular operation is mainly determined by the severity of the pathology. Among the main options for surgical treatment of malignant neoplasia of the ovary are:

 

Unilateral salpingo-oophorectomy

Surgical removal of one ovary and one fallopian tube. It is indicated in the initial stages of the disease. The operation lasts about an hour and a half. It is usually performed with a minimally invasive method through several punctures.

Bilateral salpingo-oophorectomy

Removal of both ovaries and both fallopian tubes. The intervention is usually combined with a partial or complete hysterectomy – resection of the uterus and its cervix. It lasts 1-3 hours and is performed under general anaesthesia. Bilateral salpingo-oophorectomy is also often performed for breast cancer.

Radical abdominal hysterectomy

The surgery involves fully removing the cervix, ovaries, fallopian tubes, and surrounding tissues. This procedure is performed using a low transverse incision or a vertical incision in the peritoneum. It lasts 2 to 3 hours.

Omentectomy

Removal of the affected ovary and fallopian tube along with the omentum, the thin fold of abdominal tissue covering the stomach, the colon, and other abdominal organs. Lymph nodes may also be removed during this intervention. Such a procedure is called a lymphadenectomy.

Colectomy

If cancer has spread to the intestine, it may be necessary to remove part of it. A stoma (colostomy or ileostomy) may be created to allow the intestine to function normally at a later date.
