Coxarthrosis, also known as osteoarthritis of the hip joint, is a degenerative-dystrophic disease of a chronic nature. It is characterised by the gradual destruction of the cartilage covering the femoral head and the acetabulum. Untreated, the disease can lead to disability. Thanks to modern therapies, the progression of the disease can be halted. Stem cells are one such treatment. They have shown great promise and have been very successful in controlling coxarthrosis. Osteoarthritis is predicted to become the fourth most common chronic disease leading to disability worldwide. It is a pressing international health issue. For this reason, new methods of combating the disease are constantly being developed and introduced abroad, and cell therapy is one of them.

 

Stem cells are undergoing clinical trials to treat patients with osteoarthritis of various joints. Approximately 2,390 patients have taken part in the trials. Adipose tissue stem cells (ADSC), bone marrow stem cells (BMSC) and peripheral blood stem cells (PBSC) have been used most frequently for therapy. 

 

A stem cell treatment for hip coxarthrosis was conducted at Clinica Las Condes, Santiago, Chile. It involved 10 patients aged 60 plus/minus. 

They had radiological evidence of moderate osteodegenerative disease in one or both hips. These patients had little or no pain relief with analgesics, injections of hyaluronic acid and other drugs. The results of the trial demonstrated a good regenerative effect of the stem cells.

Can coxarthrosis be cured without surgery using only stem cells?

Treatment for coxarthrosis with stem cells involves injecting cellular material directly into the affected joint. Bone marrow is aspirated (30 ml) from the posterior crest of the iliac bone to prepare the material for injection. If the patient has 2 affected hip joints, then the marrow aspiration has to be 60 ml. 

 

Once the cellular material (MSCC) has been collected, it is sent to a laboratory for the isolation and multiplication of the stem cells. The processing can take anywhere from a few days to several weeks.

 

Joint fluid is not removed before stem cell injections. All anti-inflammatory and analgesic medication needs to stop 3-5 days before the procedure.

 

During it, the stem cells are injected into the injured femur. After 7 and 14 days, the second and third doses are injected. In this way, the required number of autologous stem cells are injected into the injured femur within two weeks.

The study was conducted between May 2013 and September 2016. The results took into account the assessment of pain, stiffness, degree of impairment of physical function and range of motion of the joint. 

 

According to data from the clinical trial, in all patients, the infusion of cellular material led to a reduction in pain and improved joint function and range of motion between 16 and 40 months. In addition, radiological findings of the hip, assessed between 7 and 30 months after stem cell infusion, clearly demonstrated that the progression of osteoarthritis was halted.

Results of treating hip coxarthrosis with stem cells

Stem cells help to speed up the repair of damaged tissue and reduce inflammation. For example, intra-articular injections of mesenchymal stem cells (MSCs) show promising results. They improve joint function and reduce pain without causing serious side effects. The results last for several years.

Complications of cell therapy for coxarthrosis

    There are practically no serious side effects of the therapy after the application of stem cells. In isolated cases, patients have developed an infection after bone marrow aspiration, which has been treated with antibiotics. One patient also had a pulmonary embolism two weeks later.

    Other adverse effects of coxarthrosis cell therapy included swelling and dehydration after aspiration. A more recent evaluation of side effects showed that 4% of patients experienced injection site pain or pain syndrome associated with the progression of degenerative joint disease.
