During the interposition of the ileum, the surgeon replaces the location of the last section of the small intestine with its initial section. The procedure helps to normalize blood glucose levels and rid the patient of excess weight.

The conservative way of treating diabetes involves high hospital costs, long-term, combined medications, and no guarantees of recovery.

Metabolic surgeries help type 2 diabetes patients by:

  • establishing control over blood sugar levels, thus eliminating the need for insulin;
  • improving the function of the kidneys, eyes and liver that deteriorate in the course of the disease;
  • getting rid of excess weight (within 1-1.5 years) and high blood pressure.

Surgical treatment of diabetes is performed laparoscopically. This type of surgery is considered the safest because it excludes incisions and involves only 3 punctures with a Veress needle or a thin tube, 5-10 mm in diameter. Thus, laparoscopy provides low trauma, minimal blood loss, minimizing the likelihood of postoperative complications and a quick rehabilitation period.

On the first day after surgery, the patient is in the intensive care unit. The next day he is transferred to a normal ward. From then on, the patient may sit and walk. The patient is discharged on the 4th or 5th day. Depending on the blood sugar level, the person may need diabetes medications. They must be taken until the blood sugar level reaches normal levels. Diabetes Surgery at the Cerrahi Clinic

Cerrahi Hospital uses 2 main metabolic surgery techniques to treat diabetes: DS (Duodenal Switch) and BPD (Biliopancreatic Diversion) surgeries. The former is a transposition of the ileum, and the latter is a transient biliopancreatic division surgery.

Transposition of the ileum is a surgical technique that has proven to be safe and produces good results even in patients with type 2 diabetes with low body mass index (BMI) values.

Both techniques use a gastric tube, which allows:

  • reduce the amount of ghrelin, a substance that increases appetite;
  • increase the rate of gastric emptying, which allows food to pass into the small intestine faster and activates the receptors responsible for feeling full at an earlier stage;
  • to prevent an enlarged stomach.

Diabetes surgeries are performed under general anaesthesia. They last about 2 to 3 hours. The intestine is accessed through 5 small openings in the abdominal cavity. During the intervention, part of the stomach and intestines are removed. The small intestine is brought close to the exit hole of the stomach. 2/3 of the food entering the stomach takes this new route.

Other variations of bariatric interventions are also used to control diabetes: gastric bypass or gastric banding. Bypass surgery aims to regulate sugar using a special shunt that divides the stomach into two parts. A small intestine is attached to the smaller part of the stomach. This makes it possible to achieve satiety with minimal amounts of food.

Gastric banding involves placing a bandage on the stomach with adjustable stenosis. This method helps reduce sugar levels by allowing patients to eat less food because the bandage prevents them from eating too much.
